Case-by-Case Approvals
The key to engaging the Regional Case Manager in the coverage process is a complete Insurance Verification and Patient Education Authorization form, known as an IVEA. Compliant with HIPAA, the IVEA provides the Regional Case Manager with the patient's insurance information, medical history and statement of medical necessity to facilitate timely case by case submissions.
Case-by-case approvals and/or reimbursement have been obtained from more than 300 different payers since FDA approval in July 2005.
